I'm not entirely sure how, but normally it happens when somebody accidentally do something that's bugged and then it crashes... for example: in the server I used to play before playing here, my brother found that he was crashing the server when entering gnomeregan from the portal (the little portal machine in booty bay), he never thought he was him, 1st he just said "oh, a normal server crash, let's re-enter" at the 2nd time it was like "WTF" for him, then the 3rd time he got banned...
